Leslie Green designed the building for the Great Northern, Piccadilly and Brompton Railway, which opened the station on 15 December 1906; Barnsbury had been proposed as its name before opening. More than a century on it still relies on lifts, never having been converted to escalators. Unusually for a station of its age, those lifts go straight down to platform level with no extra stairs, which is why it can be shown as step-free on the maps without any rebuilding.

The building is Grade II listed. On the way south towards King’s Cross St Pancras, passengers can still spot the remains of York Road, the next stop until it closed in 1932. Northbound, the next station on the Piccadilly line is Holloway Road.
